A Broader View of Men’s Health

Men’s health is often discussed too narrowly.

A patient may be told his prostate is normal, his labs are fine, or his imaging does not explain his pain — yet he still has real symptoms. In other cases, a man may be given repeated antibiotics for “prostatitis” even when infection is not clearly the driver.

Pelvic and reproductive symptoms can involve several overlapping systems:

  • the pelvic floor muscles

  • the hips and deep rotators

  • the lower abdomen and adductors

  • the low back and sacroiliac region

  • peripheral nerve irritation

  • circulation and tissue sensitivity

  • stress, sleep, and nervous system regulation

  • sexual anxiety or protective muscular bracing

When these systems interact, symptoms can become persistent and confusing. That is why a more integrative and anatomically specific approach can be useful.

Acupuncture, Dry Needling, and Pelvic Floor Treatment

Dr. Yang has treated pelvic floor dysfunction and chronic pelvic pain for more than 20 years.

Treatment may include acupuncture, dry needling, myofascial assessment, lifestyle guidance, functional medicine support, and coordination with pelvic floor physical therapists, urologists, or other specialists when appropriate.

The goal is not simply to “relax the pelvic floor.” The goal is to understand why the pelvic region is irritated, guarded, painful, or functionally disrupted.

For some patients, that means addressing the hips and low back. For others, it may mean treating abdominal tension, adductor trigger points, stress-related bracing, sleep disruption, digestive issues, or a sensitized nervous system.

Male pelvic symptoms often require a clinician who is comfortable thinking across categories.

Ultrasound-Guided Dry Needling for Complex Pelvic and Musculoskeletal Pain

One of the distinctive features of Pacific Medical Group is Dr. Yang’s work with ultrasound-guided dry needling.

Traditional dry needling is usually performed by feel. Ultrasound guidance allows Dr. Yang to visualize muscles, tendons, fascial planes, blood vessels, and surrounding anatomy in real time. This can be especially useful when working with deeper or more anatomically complex regions.

For male pelvic health, ultrasound-guided treatment may be relevant when symptoms are influenced by the hips, adductors, lower abdomen, gluteal muscles, low back, or other musculoskeletal structures that refer pain into the pelvis or genital region.

Ultrasound-guided dry needling is not necessary for every patient, and it is not a cure-all. But for selected cases, it allows for a more precise and anatomically informed treatment approach.

Prostatitis-Like Symptoms and Chronic Pelvic Pain

Many men are diagnosed with prostatitis even when infection is not clearly present. In some cases, symptoms may be better understood as chronic pelvic pain syndrome, pelvic floor dysfunction, or a combination of muscular, neurological, inflammatory, and stress-related contributors.

Symptoms may include:

  • pelvic or perineal pain

  • urinary urgency or frequency

  • discomfort after ejaculation

  • groin, testicular, or penile pain

  • low back or hip tension

  • pain that worsens with sitting

  • flares during stress

At Pacific Medical Group, we do not assume that every prostatitis-like symptom is caused by the prostate itself. We look at the larger pelvic system and try to identify the actual drivers of pain and dysfunction.

Erectile Dysfunction, Sexual Function, and Pelvic Health

Erectile dysfunction and sexual performance concerns can have many causes, including vascular, hormonal, neurological, psychological, medication-related, and musculoskeletal factors.

When appropriate, we evaluate how pelvic floor tension, chronic stress, pain, sleep, circulation, and muscular guarding may be contributing to sexual dysfunction.

Some patients need medical evaluation, lab testing, or referral to a urologist. Others may benefit from acupuncture, dry needling, stress regulation, pelvic floor work, or a broader integrative treatment plan.

We approach these concerns directly, respectfully, and without embarrassment.

Reproductive Health and Fertility Support

Male fertility and reproductive health can be affected by many factors, including stress, inflammation, sleep, metabolic health, varicocele, hormonal status, medication exposure, lifestyle factors, and sperm quality.

Acupuncture and integrative care may be used as part of a broader reproductive health plan, especially when patients are trying to optimize general health, reduce stress, improve pelvic circulation, or support fertility treatment.

When needed, we encourage appropriate medical testing and collaboration with reproductive specialists.

What to Expect at Your First Visit

Your first visit will focus on understanding your symptoms, history, and goals.

We may discuss:

  • where your symptoms are located

  • when they began

  • what makes them better or worse

  • whether symptoms relate to sitting, urination, ejaculation, bowel function, stress, exercise, or sleep

  • prior diagnoses or treatments

  • medications, supplements, labs, or imaging

  • previous care with urologists, pelvic floor physical therapists, acupuncturists, or other clinicians

From there, Dr. Yang will discuss whether acupuncture, dry needling, ultrasound-guided treatment, functional medicine support, or referral to another provider makes the most sense.
